Evaluation 2009

The aim of World Hospice and Palliative Care Day ('World Day') is to raise funds and awareness of the need for hospice and palliative care across the world. Your experiences and views of the day are very important and will be used to help plan future World Days and Voices for Hospices The information you provide will be used in the World Day Evaluation Report which will be made public on the World Day Website.

 

Please take the time to complete the questions, there are five sections and it should not take more than 15 minutes. Online completed questionnaires will be submitted to Kate Tattersall at Help the Hospices.

 

You can also take part in the evaluation by downloading the form and posting it to Kate Tattersall , Help the Hospices, 34-44 Britannia Street, London, WC1X 9JG.

 

Please complete the evaluation by 31st January 2010 to ensure your event is included in the World Day and Voices for Hospices Evaluation report.
